*By purchasing a Calendar in the Ronald McDonald House Charities® of Chicagoland & Northwest Indiana 2012 Calendar Raffle for Kids (“raffle”), the purchaser acknowledges that they are at least eighteen (18) years of age, are present in Illinois at the time of Calendar purchase, and have read, understood and agreed to these rules and procedures. Calendars will be available for sale from October 10, 2011 until February 15, 2012 at midnight (CST) while supplies last. Calendars are $25 each, 3-pack for $65, 5-pack for $100 or 10-pack for $150. Only one method of payment may be used for multipack purchases. A maximum of 375,000 calendars will be sold. Only calendars that have been paid for, and have been verified as paid for, are eligible for respective draws. Calendars cannot be returned for refund once the first draw occurs. This raffle is conducted and managed by Ronald McDonald House Charities of Chicagoland & Northwest Indiana (“RMHC®-CNI”).

Who can purchase: Calendar purchasers and recipients must be individuals who are at least 18 years of age and present in Illinois at the time of purchase. No-one under the age of eighteen (18) or outside Illinois at the time the calendar was purchased can win a prize or a portion of a prize in the raffle. Calendars must be purchased and delivered within the state of Illinois. The following persons are prohibited from purchasing Calendars or obtaining any remuneration, profit, prize or benefit from the raffle: RMHC-CNI Staff, Board of Directors, Raffle Advisory Committee and all other persons with RMHC-CNI involved in the operation or management of the raffle and members of the same household of each of the above. The following persons are prohibited from purchasing Calendars or winning a prize in the raffle: employees of the Raffle Auditor, the Ticket Audit and Draw Control Auditor and members of the same household of each of the above.
How to Play: For each calendar purchased, purchasers will receive a wall calendar with an official numbered receipt. Each calendar purchased entitles the purchaser to a ticket number in the raffle. After each draw in the raffle, the winner is recorded and remains eligible for all subsequent draws. Please allow a minimum of four (4) weeks for delivery of winnings.
Prizes: 369 cash prizes will be awarded with a total value of $373,500.
Key Deadlines:
Draws: All sales will be made and finalized in the Village of Rosemont, Illinois and all raffle chances will be issued by RMHC-CNI from Rosemont. Purchasers will be accepted from throughout Illinois. Winning chances will be drawn under the supervision of the raffle audit and control firm, which will act as the third party ticket and draw control, and winners will be announced at RMHC-CNI’s Rosemont office -- 5600 N River Rd # 800 Rosemont, IL 60018-5166 -- between 8am and 5pm (CST) on the following dates:
Super VIP Prize Draw: One $10,000 prize draw on November 7, 2011. Deadline to purchase is midnight (CST) October 21, 2011.
Early Bird Prize Draw: One $25,000 prize draw on November 21, 2011. Deadline to purchase is midnight (CST) November 4, 2011.
Holiday Cash Draw: One $100,000 prize draw on January 3, 2012. Deadline to purchase is midnight (CST) December 16, 2011.
All Daily, Weekly and Monthly calendars purchased by midnight (CST) December 16, 2011 are eligible for draws commencing Jan. 1, 2012. All Daily, Weekly and Monthly calendars purchased by midnight (CST) January 15, 2012 are eligible for draws commencing Feb. 1, 2012. All Daily, Weekly and Monthly calendars purchased by midnight (CST) February 15, 2012 are eligible for draws commencing March 1, 2012. Daily, Weekly and Monthly draws falling on weekend days or holidays will be drawn on the next business day. The Daily and Monthly draws scheduled for December 29, 30 & 31, 2012 will occur on December 28, 2012.
Final Ticket Sales Deadline is: midnight (CST) Feb. 15, 2012.
Daily Prize Draws: 314 Daily prizes of $250 each will be drawn Monday through Sunday from January 1, 2012 through December 31, 2012, except on days where a weekly or monthly draw is taking place.
Weekly Prize Draws: 40 Weekly prizes of $1,000 each will be drawn every Monday from January 1, 2012 through December 31, 2012 except on days where a monthly draw is taking place.
Monthly Prize Draws: 12 Monthly prizes of $10,000 each will be drawn on the first Monday of each month from January 1, 2012 through December 31, 2012.
The location or locations at which winning chances will be determined:
Winning chances will be drawn in Rosemont under the supervision of the raffle audit and control firm, which will act as the third party raffle administrator, and will be announced in Rosemont. In certain cases, one of the larger draws (e.g., the Holiday Cash Draw) may take place at, and the winner confirmed and announced from, a public place during a celebration in Rosemont. In the case of other draws, the raffle audit and control firm will draw the winning number at its offices and the winner will be confirmed and notified from RMHC-CNI’s Rosemont office -- 5600 N River Rd # 800 Rosemont, IL 60018-5166.
Winners Notification: Winners will be posted every weekday (except holidays) on the 2012 Calendar Raffle website at www.CalendarRaffleforKids.org. All winners will be notified in writing c/o the purchaser at the mailing address provided at the time of purchase. Please allow a minimum of four (4) weeks for delivery of checks.
Taxes: The purchase price of raffle calendars does not qualify as a tax deductible charitable contribution. Each prize winner is responsible for any and all federal, state and local income or excise taxes, fees, and like charges associated with their prize(s) and for paying any such amounts. Raffle winnings are taxable income and must be reported as such on income tax returns. Prizes valued at $600 or greater will not be distributed until the Raffle Administrator has received from the winner a complete and executed IRS Form 5754 and any other documentation as may be required by the Raffle Administrator. At the time of this printing, federal law requires that the corresponding federal income tax withholding for prizes valued at $5,000 or greater must be collected from winners upon prize distribution. Thus, prizes valued at $5,000 or greater will be distributed only after an amount equal to 25% of the value of the prize is withheld or collected for the corresponding federal tax. Federal and state tax withholding laws are subject to change without notice. The withholding laws in effect at the time prizes are distributed will be followed.
+Gifting: The purchaser may give a calendar to a recipient (the giftee). All giftees must be 18 years of age or older and present in Illinois at the time of calendar purchase, and the purchaser is required to confirm this information. Each giftee should receive one official calendar and an official calendar numbered receipt. It is the responsibility of the purchaser to make certain that giftees receive their official calendar. It is also the responsibility of the purchaser to make certain that giftees receive any notifications directed to them. Prizes will be payable to giftee c/o the purchaser and sent to the purchaser’s address. It is the responsibility of the purchaser to distribute prizes to winning giftees as necessary. Calendars cannot be purchased in the name of a minor. Any requests for changes to the calendar holder's name(s) requires photo ID. Calendar purchases requiring delivery prior to December 24, 2011 must be ordered by December 2, 2011 midnight (CST).
Privacy Policy:
RMHC-CNI respects the privacy of its donors and is committed to maintaining the accuracy, confidentiality and security of your personal information. RMHC-CNI may collect personally identifying information about raffle participants for such purposes as processing your ticket request, contacting winners, or otherwise responding to your customer service requests, as well as establish, maintain, and manage our relationship with you.
RMHC-CNI reserves the right to share this personal information with Children’s Memorial Hospital as a co-beneficiary of the Raffle for the same purposes as those listed above. Additionally, RMHC-CNI may provide your personal information to reputable third-party organizations for the sole purpose of helping us fulfill your order.
Aside from the aforementioned exceptions, RMHC-CNI does not copy, reproduce, publish, post, distribute, share or sell, the name, personal contact information or description of donation or donor to any outside individual or business.
RMHC-CNI may be required to disclose your personal information to outside third parties if ordered by a local or federal court through court order, subpoena, or applicable state or federal law.

STATEMENTS - Statements made regarding the raffle are based on information available to RMHC-CNI at the time these rules were first printed.
LANGUAGE - The raffle is conducted in English.
LAWS AND REGULATIONS - All applicable federal, state and local laws and regulations are incorporated into these rules. The raffle is conducted pursuant to the Illinois Raffle Act (230 ILCS 15/0.01 et seq.) and Village of Rosemont Ordinance No. (90-12-5E).
CONDITIONS/USE RIGHTS - By accepting any prize, each winner consents to the use of their name, voice, and likeness, and all accompanying personally identifiable information the winner submits, via all forms of media now known or hereafter devised worldwide, in perpetuity, without notice, attribution or compensation (other than the prize(s) won) to the winner, his/her successors or assigns, or any other entity, except where prohibited by law.
WAIVER/LIMITATIONS OF LIABILITY - By choosing to participate in the raffle, ticket purchasers, ticket holders, and prize winners release RMHC-CNI, the Raffle Auditor and each of their respective parent companies, affiliates, partners, subsidiaries, agents, representatives, sponsors, service agencies and independent contractors, and each of their respective directors, officers, partners, employees and agents, including advertising and promotion agencies (the "released parties"), from any and all liability whatsoever, and waive any and all causes of action, for any claims, costs, injuries, losses and damages of any kind arising out of or in connection with the raffle or acceptance, possession, or use of any prize (including, without limitation, attorneys' fees, claims, costs, personal injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light, whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory. No released party shall be responsible if any prize cannot be awarded due to travel cancellations, delays or interruptions due to acts of God, acts of war, natural disasters, weather or acts of terrorism.
MISCELLANEOUS - The decisions of RMHC-CNI, Ticket Audit and Draw Control Auditor and the Raffle Auditor are final with respect to all aspects of this raffle. The raffle and these rules will be governed, construed and interpreted under the laws of the State of Illinois, without regard to choice of law principles. In the event that the operation, security, or administration of the raffle is impaired in any way for any reason, including, but not limited to fraud, virus, or other technical problems, or any condition caused by events beyond the reasonable control of RMHC-CNI or the Raffle Auditor that may cause the raffle to be disrupted or corrupted, the RMHC-CNI may, in its sole discretion, either: (a) suspend the raffle to address the impairment and then resume the raffle in a manner that best conforms to the spirit of these rules; or (b) draw winners from only the non-suspect, eligible entries received up to the time of the impairment. RMHC-CNI reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process, to be tampering with the operation of the raffle, to be acting in violation of these rules or any applicable laws relating to the raffle, or to be acting in an unsportsmanlike or disruptive manner. RMHC-CNI will resolve all disputes at its sole discretion. Any attempt by any person to undermine the legitimate operation of the raffle may be a violation of criminal and civil law, and, should such an attempt be made, RMHC-CNI reserve the right to seek damages from any such person to the fullest extent permitted by law. Any failure by RMHC-CNI to enforce any provision in these rules shall not constitute a waiver of that provision. All times listed in these rules refer to Illinois time. In the event there is a discrepancy between the winners list posted in any marketing collateral (website, print material, etc.) the Ticket Audit and Draw Control report will be considered final. If a prize is unclaimed after six (6) months, prize proceeds will be donated to RMHC-CNI.
